Considering that probably 99% of all bands go through some sort of lineup change, I was wondering if we can come up with some bands that are still around today that have NOT had any lineup changes going back to their debut album.
The first band that comes to my mind is ZZ Top.

I think they are the only one. If you want to include Country bands, you could also name Alabama.

Clutch have been around for 16 years, and with the exception of adding a keyboardist, have had the same lineup since they started. Few and far between though. King's X have had the same three since they put out their first album in 1988, but they briefly added a rhythm guitarist. Not sure if these officially count, but the original lineups are still together.

Aside from the debut album, Rush has had the same lineup for 33 years. I know that doesn't count, but it's still impressive.
